If your Wellness Portal includes an online Health Assessment, you can access the results using the instructions below. 


Depending on your wellness portal, your experience may vary slightly. 


1. Log in to your HealthyLife Wellness Portal.

2. From the Welcome page, you can click the link to Complete the Health Assessment.

3. If you are anywhere else in the wellness portal, you can also access the HA by clicking the Participate link in the navigation menu and then selecting Health Assessment from the menu.

4. Once you are on the Health Assessment page, your previous form entry will display.

5. Click the SHOW/HIDE REPORT button to toggle this form off and show your Health Assessment results.
